Once registered, you can update certain registration details through the portal, including:
- Name
- Contact details
- Residential and postal addresses
- Additional qualifications
- Fitness to Practise declarations
Accessing the Amendment Form
To amend your registration details, log in to the portal and select Amend Registration Details. You will then be presented with the Privacy Statement. From this page, you can choose to update as many or as few categories as required.
Changing Your Name
If you are changing your name, you must upload supporting documentation, such as:
- A copy of your passport; or
- Other official evidence of your name change.
Updating Address Details
You can update your residential address and add or amend a postal address if required
Qualifications
Please note that you cannot amend the physiotherapy qualification that formed part of your original registration application and was approved when registration was granted.
However, you can add details of any additional qualifications you have obtained since becoming registered.

Updating Fitness to Practise Declarations
If there have been any changes to your Fitness to Practise status, you can update your declarations in this section and upload supporting documentation where appropriate.
Declaration and Submission
Before submitting your amendment request, you will be directed to the Declaration page.
After reviewing the declarations, and your changes and select Submit.
